STATE MANDATED:
All children, ages 6 months to 59 months (approx. 5 years), attending a daycare or private pre-school program in New Jersey must receive at least one dose of Influenza Vaccine between September 1ST and December 31ST 2014.
Future Scholars must have a copy on file to prove that your child has had this immunization. Please also remember to provide us with a copy of your child's updated medical records each year.
Failure to obtain the influenza immunization for your child, in the absence of a medical or religious exemption, will result in the child being excluded from his daycare or preschool program until they receive a flu shot, or until March 31ST (the end of influenza season in New Jersey).
